Peri bottle and sitz bath after birth: how to use them, and what's normal down there
In this article
A peri bottle and sitz bath are two simple ways to keep a sore or stitched perineum clean after a vaginal birth. A peri bottle is a small squeeze bottle of warm water that you spray over the area as you pee. A sitz bath is a shallow basin of water you sit in. KKH tells new mums to wash with water and pat dry after every toilet visit. Its sheet also suggests a sitz bath for about 20 minutes, three times a day.
How to use a peri bottle, step by step
- Wash your hands, then fill a clean squeeze bottle with warm water before you sit on the toilet.
- Spray the water over the outside of your vagina as you pee. The NHS says this may ease the sting. Squatting over the toilet instead of sitting can help too.
- Rinse the whole area again when you're done, after a pee or a poo. KKH says to wash with water after every toilet visit.
- Pat yourself dry with clean tissue, gently and from front to back. KKH says to dry and dab gently. The NHS says front to back keeps germs from your bottom away from the cut.
- Press a clean pad against your stitches when you poo. The NHS says this takes some pressure off the cut.
- Put on a fresh pad. KKH advises changing pads every two to three hours.
- Wash your hands again and rinse out the bottle, so it's clean for next time.
How to take a sitz bath
A sitz bath soaks just your bottom and perineum. Cleveland Clinic says you can use a shallow bath or a small plastic basin that fits over the toilet bowl. If you have no bathtub, the basin is the one to get.
- Ask your doctor or the ward nurse before you start. Cleveland Clinic says anyone who has just had a baby should ask first and get instructions on timing.
- Clean and rinse the basin, then set it on the toilet bowl under the seat.
- Fill it with cool or lukewarm water, as KKH advises. Skip hot water: Cleveland Clinic warns it can burn your skin.
- Add salt only if your hospital told you to. KKH's sheet uses two teaspoons in the basin. Cleveland Clinic says warm water alone works, and to add salt or anything else only as your doctor directs.
- Lower yourself in until the water touches your perineum. KKH suggests staying for about 20 minutes, three times a day.
- Pat dry with a clean towel, without rubbing. Then empty the basin, clean it and keep it for your own use.
- Stop and call your doctor if the pain gets worse or the area turns red and puffy, as Cleveland Clinic advises.
A sitz bath can help piles too. Cleveland Clinic says it can ease their pain and swelling. Our guide to piles after giving birth covers the rest.
What's normal for your vagina and perineum after birth
Expect some soreness and swelling down there after a vaginal birth. Cleveland Clinic notes that the perineum may be sore and swollen after childbirth. The NHS tips in the steps above help with the sting when you pee.
You'll also bleed for a few weeks. KKH says this discharge, called lochia, turns from bright red to pink or brown, and most mothers find it stops within three to four weeks. Our postpartum bleeding guide covers what to expect week by week.
If you're breastfeeding, your vagina may feel dry. KKH doctors say lower oestrogen while you breastfeed can cause dryness and painful sex. They suggest a lubricating gel until your own moisture returns. Our guide to sex after giving birth covers when to start again.
If things feel looser, or you leak a little pee when you laugh or cough, pelvic floor exercises help. HealthHub says to squeeze the muscles you'd use to hold back urine, and to see your doctor if the exercises haven't helped after three months. Our pelvic floor guide shows how to do them, and prolapse after childbirth covers a heavy or dragging feeling.
| What you notice | Usually normal | See a doctor if |
|---|---|---|
| Soreness | Sore to sit at first, easing over a few weeks | Pain is severe or getting worse (KKH) |
| Swelling | Some swelling of the perineum (Cleveland Clinic) | Red, swollen skin around the stitches (NHS) |
| Stinging when you pee | Eases with warm water as you pee (NHS) | You also have a fever or chills (KKH) |
| Discharge | Red, then pink or brown (KKH) | Pus-like or foul-smelling (KKH) |
| Stitches | Dissolve on their own (KKH) | The wound bleeds or looks open (KKH) |
| Dryness | Can happen while breastfeeding (KKH doctors) | Sex still hurts at your postnatal check |
| Leaking pee | Small leaks when you laugh or cough (HealthHub) | Exercises haven't helped after three months (HealthHub) |
Cold packs, rest and pain relief
For swelling, try something cold. KKH's list includes pads with cooling gel, and the NHS suggests an ice pack or ice cubes wrapped in a towel. Don't put ice straight on your skin, the NHS adds, as it can cause damage.
Rest takes the weight off. KKH says to lie down or rest on your side every few hours, and to avoid sitting for long periods. The NHS says fresh air helps the stitches heal. Take off your underwear and lie on a towel for about 10 minutes, once or twice a day.
For pain, take the painkillers your hospital prescribed, as KKH advises. Before you buy anything from a pharmacy, ask the pharmacist or your doctor, and tell them you're breastfeeding.
How long until it feels normal
For most women the soreness fades over a few weeks. KKH says a perineal tear usually heals in two to three weeks, and that the size of the cut and the type of stitches make a difference. The stitches dissolve on their own, KKH adds, so nobody needs to take them out. The NHS says it's unusual for episiotomy pain to go on longer than 2 to 3 weeks, so get it checked if yours does.
Your doctor checks the area at your postnatal review. HealthHub says it's usually 4 to 6 weeks after delivery, when the doctor examines your vagina, cervix and uterus to make sure you're healing well. KKH books the appointment before you go home, at its Women's Specialist Clinic, a GP clinic or a polyclinic.
Our episiotomy and tear guide has the healing timeline week by week. The six-week postnatal check guide lists questions to bring.
Herbal baths and confinement
Many families prepare herbal baths during confinement. That's a family custom, and HealthHub says bathing in warm water is encouraged for hygiene and comfort in the hot weather.
Keep your sitz bath separate. Cleveland Clinic says to add anything to it only as your doctor directs. KKH asks you to check with a doctor before using any traditional or complementary medicine. So on fresh stitches, use plain water, or salt if your hospital said so. Our guide to showering during confinement covers herbal baths and hair washing.

When to see a doctor
Call your gynae's clinic or the hospital where you delivered if you have any of these:
- Pus-like or foul-smelling discharge from your vagina (KKH)
- Bleeding from your stitches (KKH)
- A fever or chills (KKH)
- Perineal pain that is severe or getting worse (KKH)
- Red, swollen skin around the cut or tear, or stitches that hurt more (NHS)
- A wound that seems to have opened. KKH doctors say tears can break down from poor healing and infection
- Pain that hasn't eased after 2 to 3 weeks (NHS), or isn't better after two or three sitz baths (Cleveland Clinic)
- Bleeding that suddenly gets heavy after it had eased (KKH)
KKH's Urgent O&G Centre is a 24-hour walk-in centre at Basement 1 of the Women's Tower, and it sees post-pregnancy problems. KKH also takes same-day bookings there, and walk-ins stay open for urgent care. If you need immediate medical attention, call 995, as NUH advises. Our postnatal recovery guide has the full list of warning signs after birth.
Frequently asked questions
How to use a peri bottle after birth?
Fill it with warm water and spray it over the outside of your vagina as you pee. Rinse again when you're done, then pat dry gently from front to back. KKH says to wash the area with water after every toilet visit and change your pad every two to three hours.
How to take a sitz bath after birth?
Ask your doctor first. Then fill a clean basin on the toilet with cool or lukewarm water and sit so it touches your perineum. KKH suggests about 20 minutes, three times a day. Add salt only if your hospital told you to.
Is a sitz bath safe if I have stitches?
KKH includes a sitz bath in its own care advice for an episiotomy or tear. Keep the basin clean and for your use only, and skip hot water. If the area gets redder or more painful, stop and see a doctor, as Cleveland Clinic advises.
What does your vagina feel like after giving birth?
Sore and swollen at first, with bleeding that turns from red to pink or brown. If you're breastfeeding it may feel dry, which KKH doctors link to lower oestrogen. Pelvic floor exercises help if things feel looser or you leak a little pee.
What is the episiotomy healing time?
KKH says a perineal tear usually heals in two to three weeks, though a bigger cut can take longer. The NHS says pain that goes on past 2 to 3 weeks is unusual and worth a doctor's check. Our episiotomy guide has the full timeline.
How long do I need perineal care after birth?
Keep up the rinsing and fresh pads until your stitches have healed and it no longer hurts to pee. For most women that's a few weeks. HealthHub says your doctor checks the area at your postnatal review, usually 4 to 6 weeks after delivery.
