Most women find out at 5 weeks pregnant. KKH's planner puts the likeliest discovery right here, and a home kit picks up the pregnancy hormone hCG 99% of the time once it's high enough. Inside, your baby's organs have started to form, from three thin layers of cells that will each become a different set of body parts.

This week you'll want to confirm the pregnancy, book a first visit, and know which kind of bleeding needs a same-day check.

Your baby at 5 weeks: three layers that become everything

KKH describes the coming weeks as a sensitive period when cells sort out where each organ will go. By the end of week 5 they have arranged themselves into three germ layers:

  • the ectoderm, the outer layer, which becomes the outer skin, the backbone, the brain and the nerves
  • the mesoderm, the middle layer, which becomes deeper skin, bones, muscles, the heart and blood vessels, and the immune system
  • the endoderm, the inner layer, which becomes most of the gut, the liver and pancreas, the urinary system and the lungs

One end of the embryo is set to become the head, the other the tail end. HealthXchange notes that the brain, heart, lungs and intestines all begin to develop in week 5, and KKH says every organ system is formed, though immature, by week 10.

Confirming the pregnancy

A positive home test is usually enough to book a doctor; our positive pregnancy test step list covers who to see and what to stop. A blood test for hCG is more accurate, KKH says, and measures the actual level. Doctors sometimes repeat it, because the pattern of change can flag an ectopic pregnancy or a miscarriage early.

A doctor's confirmation matters at work too. MOM's maternity protection covers you against dismissal without sufficient cause, and against retrenchment, during pregnancy. You qualify if you've worked for your employer for at least 3 months and a medical practitioner certified the pregnancy before any notice was given. MOM's notice rule is about the leave itself, at least 4 weeks before it starts, so there's no deadline to tell your employer yet.

What you might feel

Often very little. KKH lists tiredness, light-headedness and light bleeding, sometimes with cramping, and notes these feel a lot like a period about to start. Morning sickness usually peaks later in the first trimester, so keep our morning sickness remedies handy for weeks 6 to 10.

KKH's list for this week is short: keep taking folic acid, skip raw food and unpasteurised dairy to avoid stomach infections, keep exercise to moderate intensity, and stop smoking and drinking. Our guide to foods to avoid during pregnancy has the Singapore list.

For subsidised care at a public hospital, start with a polyclinic or a CHAS GP clinic, which can refer you on; MOH ties specialist outpatient subsidies for citizens and PRs to those referrals. For private care, book your gynae directly. The delivery cost calculator shows how each choice adds up.

Bleeding at 5 weeks: when to go in

KKH says light bleeding at this stage is usually harmless, but it can also be a sign of miscarriage or an ectopic pregnancy, and you should get advice if you're unsure. Go the same day for:

  • bleeding with cramps or tummy pain
  • severe pain low in your tummy
  • feeling faint or dizzy
  • chest pain or shortness of breath

At KKH, early pregnancy problems such as bleeding and tummy pain go to the Urgent O&G Centre, a 24-hour walk-in centre at Basement 1 of the Women's Tower. For minimal bleeding or mild pain under 16 weeks, KKH's Early Pregnancy Assessment Unit offers appointments the same or next working day. Booked elsewhere? Call that hospital's O&G clinic, or go to its emergency department after hours.

Frequently asked questions

Is it normal to have no symptoms at 5 weeks pregnant?

Yes. KKH describes week 5 symptoms as vague and non-specific, and morning sickness usually peaks later in the first trimester. Our guide to having no pregnancy symptoms explains when to book a scan.

Can you see the baby on a scan at 5 weeks?

Usually not much yet. HealthXchange says the heartbeat can be seen on a transvaginal scan from week 6, and HealthHub says the dating scan can be scheduled anytime from week 6 to 12.

5 weeks pregnant is how many months?

Just over one month, counted from the first day of your last period.

Should I go to a polyclinic or straight to KKH?

For subsidised hospital care, a polyclinic or CHAS GP referral comes first. For pain or bleeding, KKH's Urgent O&G Centre takes walk-ins at any hour.

Week by week. Previous week: 4 weeks pregnant. Next week: 6 weeks pregnant. Every week from 4 to 40: pregnancy week by week.
